The neck is an area which is seriously overlooked when it comes to skincare and sun protection – but it’s one of the first places that reveals the true signs of ageing ‒ and is the most difficult to achieve fast repairing results. Dr Sly Nedic reports on the latest nonsurgical laser procedure to treat a sagging neck.

Neck skin is the most delicate area after the eye area. It’s also thinner – and has the least amount of supportive elastin content. All this makes neck skin more vulnerable to free radicals and sun damage. In light of the above, it surprises me that majority of patients disclose they never put sun block on the neck – or even a daily moisturizer for that matter! That said though, even the ones who do use sun block and moisturizer on the neck area cannot stop the inevitable signs of ageing, without help from aesthetic physicians. With neck ageing (as in the other parts of the body), collagen and elastin fibers weaken – and with the gravitation effect, the delicate skin becomes more lax. In addition, infrared from sun exposure, sun beds, indoor infrared light etc, further damage the collagen and elastin. The neck area also doesn’t have any fatty tissues, which makes the skin less elastic too. The years of harmful UV effects and free radical formation (which is often genetically predisposed), affects the skin appearance as well. As a result, the skin in this delicate zone can become far more wrinkled, saggy, crepey (tiny vertical lines) and pigmented, than that on our face.

Through the whole neck runs a muscle (platysma) that does not have fixed attachments (such as the other muscles that attach to ligaments and bone). The platysma is almost free-floating in the neck, and is actually pulled forward by its own contraction. As the ageing sets and the platysma becomes more lax, the skin and connective tissue beneath loses its tone – so the neck as a unity appears to sag. Furthermore, as we grow older the platysma muscle shortens, contracts and becomes fibrose – which creates two bands in front portion of the neck, called platysma bands . Complex neck ageing reveals many conditions: sagging skin, wrinkles, vertical crepey lines, sun damaged skin appearance with pigmentation, and lastly, platysma bands. With all these indications, it’s no wonder the ageing neck is nick-named the ‘turkey neck’ – and is every woman’s bane.

Treatment modalities
Complexity of neck ageing will need combination treatments for best results. We used to combine Radiofrequencies like Tripollar® or Thermage ® – or infrared tailored light such as Titan® for neck sagging. These treatments target collagen mostly, where new collagen production is enhanced. Furthermore, we used IPL for pigmentation and sun damaged skin – as well as ablative lasers for wrinkled looking skin. However, this had huge limitations as it could not be used in our darker skin types (Fitzpatrick V-VI) – and could also only be used with special care. We also used cryo-mesotherapy to enhance elastin production for neck crepey lines. While this procedure delivered good results, it was only visible after the 6th treatment – which needless to say, did not satisfy the many patients who demand to see immediate results. All these treatments have their unique place in various aesthetic rejuvenations and makeovers – but when it comes to the neck area, we now have advanced laser technology where we are able to achieve all these improvements with one device.
